No additional fees, copays or deductibles for any visits with Dr. Gunn.
Includes on-call telephone triage and evaluation. Office, telemedicine or home visits for ~ $2400 per 12 month year, up to 4 visits per month as deemed medically necessary by Dr. Gunn.
Additional adult member may be added for $100 per month - $1200 per 12 month year
Additional children under the age 26 may be added for $62.50 per month - $750 per year for each child.
These fees include necessary comprehensive physical and history lab orders, radiology orders, referrals as needed and a written summary of recommendations.
There is no additional fee for each office/telemed visit, pre-op visit or annual physical.
Visits to you at the hospital available at doctors discretion for $250 supplement per day.
Visits to you at your home available at doctors discretion for $250 supplement.

For all Annual membership Direct Primary Care patients who have a child under the age of 26, they can add their children and receive the same benefits for only $750 per year.
For all Complete Direct Primary Care patients who have a spouse, they can add their spouse and receive the same benefits for $1200 per year.

For patients WITHOUT an annual membership, who just wish to have their one-time annual physical visit, this can be done at the office for $1200, or at your home or office for a $500 supplement.
This visit includes your lab order, radiology order, referrals as needed, written summary of recommendations, as well as video/telephone review of findings for no additional fee.

An intervention at your home, or in office consultation regarding issues surrounding the use of addictive substances. Including alcohol, tobacco, prescription medications, marijuana, and other drugs of abuse at office. This consult includes evaluation for level of care needed, and if appropriate for outpatient management, prescription, laboratory and referral for psychiatric care. Also includes 30 days of follow up care for $1200.
If not appropriate for outpatient treatment, a detailed list of options for referral for a higher level of care will be given (psychiatrist, intensive outpatient, sober living, residential or inpatient hospitalization, methadone clinic).
Medication assisted treatment, including naltrexone, Vivitrol suboxone/subutex, buprenorphine for select patients who qualify. Not all patients will qualify, and this is at the doctors discretion. Monthly MAT in office or via telehealth visits $350.

Dr. Gunn is available to visit you at your home, office or assisted living facility. For established patients the fee is $250, and $450 for a post-hospital transition visit. For a new single visit patient, the fee is $500. Distance must be near 15 miles of the office, or discussed in advance otherwise.

Single visit evaluation and management of on-going chronic conditions for $250 at office, via telehealth or telephone. At your home, $500 for non-membership patients. Membership patients have discounted rate.
Visit to your home, at doctors discretion. Must be within 15 mile radius of Westlake Village Blvd. and Triunfo Canyon, 91361. Outside of this area for supplemental fee and at Doctors discretion.
Includes prescription renewal, lab and radiology orders, referrals as necessary. Includes any necessary follow up telephone or telehealth for review of abnormalities found on testing.

Same day evaluation of urgent condition, treatment if appropriate or detailed instructions of appropriate care at urgent care or hospital as appropriate with assisted hand-off via telephone and written report. Single visit at office or via telemedicine is $250. Visit to your home available for $500.

Hospital Discharge follow-up; Admission to Assisted Living Facility/SNF; Pre or Post-surgical Evaluation all available at the rate of $350/visit. Visit to home available for $500, or $250 for established membership patients.

Conejo Valley Direct Primary Care provides the best quality service without the bureaucracy of a large health care system - you have direct access to your doctor at a time convenience to you. Because of this format, Dr. Gunn is out of network for insurance and does not accept insurance as a form of payment. However, the quality of care is exceptional and you will appreciate the convenience and attention to detail he provides.
Annual membership can be paid for all at once, semi-annually, quarterly or monthly. Pay all at once and get 13 months for the price of 12!
All major forms of payments (credit card, venmo, check, cash, etc…) are accepted at time of service. Credit card use will incur 4% fee. Use personal check, zelle, or cash to avoid this fee.
